• This offer is available to all active members of PDCA.
• Only one referral claim accepted per company/member.
• Referrals must be submitted using the, “PDCA Membership Stimulus Package” referral form.
• Referred new members become eligible to refer other members after activation of their membership.
• This offer may not be combined with other PDCA Headquarters’ promotions. 
• Referral forms must be received no later than December 31st 2009.

• Gift certificates are to be used toward the referring company’s next membership renewal. 
• Gift certificates expire on 12/31/2010.  Unused gift certificates will not be accepted/honored after that date.
• Referral gift certificates will be mailed to recipients upon activation of the referred company’s membership.
• Referral Gift Certificates will not be issued for National Trial Membership applicants.
